Fulminantmyocarditis (FM) is a form of acute myocarditis characterized by severe left ventricular systolic dysfunction requiring inotropes and/or mechanical circulatory support. A single-center study found that a patient with FM had better outcomes than those with acute nonfulminant myocarditis (NFM) presenting with left ventricular systolic dysfunction, but otherwise hemodynamically stable. This was recently challenged, so disagreement still exists.
Objectives
This study sought to provide additional evidence on the outcome of FM and to ascertain whether patient stratification based on the main histologic subtypes can provide additional prognostic information.
Methods
A total of 220 patients (median age 42 years, 46.3% female) with histologically proven acute myocarditis (onset of symptoms <30 days) all presenting with left ventricular systolic dysfunction were included in a retrospective, international registry comprising 16 tertiary hospitals in the United States, Europe, and Japan. The main endpoint was the occurrence of cardiac death or heart transplantation within 60 days from admission and at long-term follow-up.
Results
Patients with FM (n = 165) had significantly higher rates of cardiac death and heart transplantation compared with those with NFM (n = 55), both at 60 days (28.0% vs. 1.8%, p = 0.0001) and at 7-year follow-up (47.7% vs. 10.4%, p < 0.0001). Using Cox multivariate analysis, the histologic subtype emerged as a further variable affecting the outcome in FM patients, with giant cell myocarditis having a significantly worse prognosis compared with eosinophilic and lymphocytic myocarditis. In a subanalysis including only adults with lymphocytic myocarditis, the main endpoints occurred more frequently in FM compared with in NFM both at 60 days (19.5% vs. 0%, p = 0.005) and at 7-year follow up (41.4% vs. 3.1%, p = 0.0004).
Conclusions
This international registry confirms that patients with FM have higher rates of cardiac death and heart transplantation both in the short- and long-term compared with patients with NFM. Furthermore, we provide evidence that the histologic subtype of FM carries independent prognostic value, highlighting the need for timely endomyocardial biopsy in this condition.
Background: Lymphocytic myocarditis causes left ventricular dysfunction that may be persistent or reversible. There are no clinical criteria that predict which patients will recover ventricular function and which cases will progress to dilated cardiomyopathy. We hypothesized that patients with fulminant myocarditis may have a better long-term prognosis than those with acute (nonfulminant) myocarditis.
Methods: We identified 147 patients considered to have myocarditis according to the findings on endomyocardial biopsy and the Dallas histopathological criteria. Fulminant myocarditis was diagnosed on the basis of clinical features at presentation, including the presence of severe hemodynamic compromise, rapid onset of symptoms, and fever. Patients with acute myocarditis did not have these features. The incidence of the end point of this study, death or heart transplantation, was ascertained by contact with the patient or the patient’s family or by a search of the National Death Index. The average period of follow-up was 5.6 years.
Results: A total of 15 patients met the criteria for fulminant myocarditis, and 132 met the criteria for acute myocarditis. Among the patients with fulminant myocarditis, 93 percent were alive without having received a heart transplant 11 years after biopsy (95 percent confidence interval, 59 to 99 percent), as compared with only 45 percent of those with acute myocarditis (95 percent confidence interval, 30 to 58 percent; P=0.05 by the log-rank test). Fulminant myocarditis was an independent predictor of survival after adjustments were made for age, histopathological findings, and hemodynamic variables. The rate of transplantation-free survival did not differ significantly between the patients considered to have borderline myocarditis and those considered to have active myocarditis according to the Dallas histopathological criteria.
Conclusions: Fulminant myocarditis is a distinct clinical entity with an excellent long-term prognosis. Aggressive hemodynamic support is warranted for patients with this condition.

Case report
We describe a case of vaccine-associated measles in a two-year-old patient from British Columbia, Canada, in October 2013, who received her first dose of measles-containing vaccine 37 days prior to onset of prodromal symptoms. Identification of this delayed vaccine-associated case occurred in the context of an outbreak investigation of a measles cluster.
In this report we describe a case of measles-mumps-rubella (MMR) vaccine-associated measles illness that was positive by both PCR and IgM, five weeks after administration of the MMR vaccine. Based on our literature review, we believe this is the first such case report which has implications for both public health follow-up of measles cases and vaccine safety surveillance.
Between 29 August and 2 September 2013, three unlinked persons from across the Fraser Valley, British Columbia, Canada, presented with rash illness consistent with clinical measles [1]. Based on the outbreak investigation by the local health authority, none of the three cases had an identified exposure to a measles case or travel history outside of Canada during the incubation period, and a source case was never identified. All three cases had the same measles genotype B3 sequence type (MVs/British Columbia.CAN/34.13, MeaNS id 39928, GenBank accession numbers KF704002 and KF704001). Measles genotype B3 is endemic in the World Health Organization’s African and Eastern Mediterranean regions [2]. Two additional cases of measles due to secondary transmission from one of the above cases were identified in British Columbia in the third week of September.

Abstract
An orthopoxvirus was isolated from the vesicular rash of a man in Natal who died in coma and who had not been vaccinated. Analysis of the viral DNA showed that it was a vaccinia virus, more closely related to the virus of South African smallpox vaccine than to other vaccinia viruses. DNA analysis also showed that an orthopoxvirus isolated from a sporadic case of severe pustular rash in Nigeria was a vaccinia virus closely related to the smallpox vaccine virus used there. Minor biological differences between the viruses isolated and the corresponding vaccine strains suggested that some natural transmission of the virus had occurred, but the results of DNA analysis implied that they originated from the use of smallpox vaccine. No similar cases have been detected since smallpox vaccination was discontinued.

Abstract
The authors report the case of a 26 year old woman with acute anterior poliomyelitis contracted during the vaccination of her baby. Despite having been herself vaccinated in infancy she was not protected against the poliovirus. The clinical interest of this uncommon case is a severe paralytic state with definitive paraplegia. The authors suggest serologic testing of patients born before 1967 especially if they are at risk of encountering the virus.

Western Australia’s chief health officer says there are now 251 confirmed cases in the state of children aged under five having adverse reactions to flu vaccinations.
Last week the Health Department placed a temporary ban on flu shots for children under five after reports of children getting sick after being vaccinated.
The department says 55 children have had convulsions after being vaccinated while nearly 200 others have suffered fever and vomiting.
One child is in a serious but stable condition at Princess Margaret Hospital.

Me Hartemann, leur avocat, a plaidé leurs dossiers, vendredi dernier, devant le Tribunal de Bobigny.
A l’issue d’une heure trente de plaidoirie, Me Hartemann se dit « plutôt agréablement surpris » par le verdict de l’audience : les laboratoires ne s’opposent pas au lancement d’expertises médicales pour quatre des cinq dossiers. Un premier feu vert qui, s’il est confirmé, ouvrira la voie à des examens complémentaires et à la recherche de maladies rares chez ces enfants qui, après avoir reçu des injections de vaccins ont présenté des troubles neurologiques graves. « Des séquelles comparables à des traumatismes crâniens ou à des épilepsies énormes qui auraient endommagé le cerveau », précise l’avocat. Il s’agit d’enfants qui ne présentaient aucun problème de santé », poursuit-il en insistant sur la similitude des histoires, «et qui, suite à la première injection ou au rappel, ont d’un seul coup arrêté d’évoluer et présenté des dommages très importants ».
The new cases are a vaccine-derived poliovirus type 2, a rare type which can emerge in under-immunised communities after mutating from strains contained in the oral polio vaccine.
“Such vaccine-derived strains tend to be less dangerous than wild polio virus strains, they tend to cause less cases, they tend not to travel so easily geographically. That’s all kind of the silver lining and should play in our favor operationally,” he said.
All polio strains can paralyze within hours.
Syria is one of the last remaining pockets of the virus worldwide. The virus remains endemic in Afghanistan and Pakistan.
Vaccines tested on Australian orphans

An outcry erupted in Australia yesterday over revelations that hundreds of orphan babies and small children were used as guinea pigs in experiments on vaccines for herpes, whooping cough, influenza and other diseases, for 25 years after the Second World War.
Former wards of state demanded a judicial inquiry after it was disclosed that some of the tests did not work, failed to pass safety tests in animals and caused vomiting, abscesses and other side-effects in babies.
The revelations came in a report in the Age newspaper of Melbourne, which outlined seven separate cases of such experiments taking place in orphanages in Victoria state between 1945 and 1970. The experiments were conducted under the auspices of the Walter and Eliza Hall Institute of Medical Research, one of Australia’s most prestigious research institutions, and the Commonwealth Serum Laboratories, then a federal government body.
They took place without the consent of any of the children’s parents at a time when orphanages were crowded with state wards under an official policy that children from poor families and single parents should be placed under the care of the state. Such policies continued until the 1970s.
Michael Wooldridge, the federal minister for health, said last night that the experiments should never have happened. He said they were conducted according to the medical ethics of the day and should not be judged by today’s standards. “We will do everything we can to put people’s minds at rest.”

Thousands of parents have protested the new law this past June 3rd, protests and marches have taken place in 21 Italian cities spread across the nation. A national protest is scheduled for this Sunday June 11th.

Family to Receive $1.5M+ in First-Ever Vaccine-Autism Court Award

The first court award in a vaccine-autism claim is a big one. CBS News has learned the family of Hannah Poling will receive more than $1.5 million dollars for her life care; lost earnings; and pain and suffering for the first year alone.
In addition to the first year, the family will receive more than $500,000 per year to pay for Hannah’s care. Those familiar with the case believe the compensation could easily amount to $20 million over the child’s lifetime.
Hannah was described as normal, happy and precocious in her first 18 months.
Then, in July 2000, she was vaccinated against nine diseases in one doctor’s visit: measles, mumps, rubella, polio, varicella, diphtheria, pertussis, tetanus, and Haemophilus influenzae.
Afterward, her health declined rapidly. She developed high fevers, stopped eating, didn’t respond when spoken to, began showing signs of autism, and began having screaming fits. In 2002, Hannah’s parents filed an autism claim in federal vaccine court. Five years later, the government settled the case before trial and had it sealed. It’s taken more than two years for both sides to agree on how much Hannah will be compensated for her injuries.
Whooping Cough Outbreak In Alabama Spread By Vaccinated Children

The Alabama Department of Public Health has confirmed that an outbreak of pertussis – also known as whooping cough – in the state was caused by children recently vaccinated against the contagious illness.
The number of people with confirmed whooping cough has risen to 19 in an outbreak linked to local schools in Chambers County, where it spread from recently vaccinated to children to adults.
The Alabama Department of Public Health began investigating the outbreak earlier this month after 6 students came down with the highly-contagious illness. All 19 of the infected adults and children in Chambers County with whooping cough received the pertussis vaccination.
AL.com reports: Dr. Karen Landers, assistant state health officer, said the pertussis vaccine is not 100 percent effective. Infants receive several doses during their first year of life, and boosters in early and late childhood. Doctors also recommend pertussis vaccines to pregnant women and unvaccinated adults.
The illness is particularly dangerous for babies younger than 12 months old who have not received the full course of vaccinations. Complications can lead to hospitalization and even death.
However despite the recent outbreak of potentially deadly pertussis caused by the pertussis vaccine, Landers said the best way to protect babies is still to vaccinate them and ensure all adult caretakers have also received vaccinations.
